The 1987 capsize of the UK ro-ro/passenger ferry Herald of Free Enterprise due to the bow doors being left open when the ship went to sea. The Formal Investigation identified a lack of a safety culture in both the shore-side and shipboard management, and its report found that “from top to bottom the body corporate was infected with the disease of sloppiness”.
