What action should a master take if, on joining a ship that is not due for a Safety Equipment survey for another 6 months, he finds minor defects in the LSA (e. g. a dead lifebuoy light and missing reflective tape)?

Raise a non-conformity in accordance with the Company’s SMS procedures. Either make good the defects before sailing or apply to the MCA for a general inspection and get the MCA’s written approval to sail.
