To: (1) give medical care in accordance with the Ship Captain’s Medical Guide;Β 

(2) record the illness in the Official Log Book, stating (a) the occurrence of the illness, (b) the treatment given, (c) the progress of the illness and (d) the disposal of the case (i. e. whether he was cured); (3) notify the port health authority of a case of infectious disease on board before arrival at the next port of call; (4) complete a Maritime Declaration of Health if required (see Section I-Health Clearance Procedures).
