(1) Everyone, from senior management to new recruits, knows exactly what their responsibilities are, because company procedures and instructions are fully documented; (2) continuous safety training is provided; (3) there is a “no- blame” culture, so that personnel are encouraged to report accidents, near- misses and deficiencies without fear of retribution; and (4) there is no “passing the buck” when something goes wrong.
