What is shown in the Appendix to a Dangerous Goods Document of Compliance?

A small side elevation of the ship showing the under-deck spaces, and a plan view showing the on-deck spaces, intended for carriage of dangerous goods; 

and a table showing, for each class of dangerous goods (other than classes 6. 2 and 7), the spaces where the goods will be carried and a letter to indicate whether packaged goods are permitted (letter “P”), packaged and bulk goods are allowed (letter “A”), or dangerous goods are not allowed (letter “X”). Cargoes in bulk may be listed individually and by class.
