(1) The Company or the master must complete a form entitled “Amendments to the Continuous Synopsis Record (CSR) Document Number… for the Ship with IMO Number………. sometimes called CSR Form 2 or the CSR Amendment form.
(2) The original of the CSR Form 2 (Amendment form) must be attached to the current CSR Document pending issue of a new CSR Document by the flag State Administration. (3) A copy of the completed CSR Amendment form must be forwarded without delay to the flag State Administration for their consideration and action. (4) Details of the amendment(s) must be entered in the CSR Index of Amendments.
