Pilot card; General Declaration (IMO FAL form 1); Ship’s Stores Declaration (IMO FAL form 3); Crew’s Effects Declaration (IMO FAL form 4); Crew List (IMO FAL form 5); if a cargo vessel is carrying 12 passengers or fewer: a Passenger List (IMO FAL form 6); Cargo Declaration; statutory and class certificates (preferably in clear plastic pockets in a binder); customs clearance from last port; Maritime Declaration of Health; written authorisation for ship’s agent to act for the master; and (if required) Declaration of Security.
