Whose signatures should be on a Permit-to-Work?

In all cases, those of the authorising officer and the person responsible for carrying out the specified work. If the authorising officer formally transfers responsibility to another authorised person, either as a matter of routine or in an emergency, that person should also sign the permit to indicate transfer of full responsibility.
