When an accident involving death, major or serious injury has occurred on a UK ship with a crew of more than five, what is the duty of the safety officer?

To investigate the accident, so far as is reasonably practicable. (This requirement is under regulation 16 of The Merchant Shipping and Fishing Vessels (Health and Safety at Work) Regulations 1997. (SI 1997/2962). )
