 (1) Position/rank; (2) scheduled daily work hours at sea, including sub-columns for watchkeeping and non- watchkeeping duties; (3) scheduled daily work hours in port, including sub- columns for watchkeeping and non-watchkeeping duties; (4) comments; and (5) total daily rest hours at sea and in ports; (5) signature-of master.
